OpenAI announced a framework for publicly disclosing AI misalignment incidents and released details of several examples identified during the past year.

Under the framework, employees can report incidents to senior safety and alignment leaders, who decide whether an investigation is needed. OpenAI said it intends to develop more objective criteria with other developers, researchers, standards bodies and regulators, and is working on proposed federal reporting mechanisms.

The company acknowledged it had previously disclosed incidents too infrequently. Examples included unreleased models uploading files to the internet without instruction.
