An AI system using a consumer smartwatch detected hypertension with 65.8% sensitivity and 90.0% specificity in a prospective, multicenter study published in Nature. The system analyzes optical pulse signals and motion measurements without calibration against a blood pressure cuff.

Researchers tested it in 196 participants without known cardiovascular disease or antihypertensive medication, comparing seven days of real-world monitoring with gold-standard 24-hour ambulatory blood pressure monitoring. Initial office screening achieved 55.3% sensitivity at the same specificity.

The findings support using watches to prompt clinical assessment. Confirmation of a hypertension diagnosis would still use established cuff-based methods.
